UnitedHealthcare Basics PPO Plan 152

UnitedHealthcare Basics plan gives you the freedom to receive professional care from the UHC network, including specialists, without a referral. You may also choose to seek care outside the network, without a referral. You have access to more than 520,000 doctors, 4,700 hospitals and 56,000 clinical pharmacy services nationwide. Benefits include:

The Denex Dental PPO Plan is specifically designed to give each member the “choice” and flexibility to visit any licensed provider. However, benefit amounts will generally be higher if members receive care In-Network. The Network has 70,000 participating providers, general dentists and specialists, to provide services. Subscribers enjoy the advantages of:

  • Discounted fees for service.
  • Providers completing and submitting claim forms on your behalf.
  • No balance billing.
  • No referrals required to visit a specialist.

Advantica EyeCare

A Subsidiary of Allied EyeCare, LLC, Advantica EyeCare Inc. has a national network of providers where members can obtain benefits, including For Eyes Optical, Sears Optical, JC Penney Optical, Target Optical, Pearle Vision Centers, and The EyeCare Centers of America including VisionWorks.

Benefits require no authorization, are unlimited and include:

  • Comprehensive eye examination. Subscribers and covered dependents can receive one covered eye exam every twelve month period from an independent eye care doctor. Eye examinations include testing for near or distance vision in addition to assessing the overall health of the eyes.
  • Contact lens examination.Includes contact lens fitting and follow up care for soft, spherical daily wear contact lenses.
  • Frame prices. All eyewear will be available to Advantica EyeCare members at a 15% discount . A large selection of frames is available including an extensive collection of brand name and designer frames.
